Important Information for Your First Appointment

Thanks for contacting us and for your interest in our professional services in Clinical Psychology.  We welcome you to our practice, and will gladly assist you.  This document contains important information about our professional services and business policies.  Please read it carefully and ask any questions you might have at any time.  When you sign this document, it will represent an agreement between us.

Your appointment time, date, and office location will be given to you at the time of your request, and you will be sent an e-mail confirmation.  Your appointment may be via telepsychology or in person.

If via telepsychology, please arrive to the virtual waiting room at least a few minutes before the time of the appointment, and plan to be with us for about one hour.

During the COVID-19 pandemic, our physical offices are closed, and we are limited to providing our professional services in Clinical Psychology via telepsychology (sessions by telephone or encrypted video).  Furthermore, all communications from us will be via encrypted e-mail, as it is considered a safe method to communicate Personal Health Information (PHI).

Please note that we are ethically and professionally limited to providing TelePsychological services in Ontario, Canada, and in California, U.S.A., where we are registered/licensed as Psychologist.  To ascertain your eligibility for our services, please send us proof of where you live (e.g., copy of valid driver’s license or other valid government issued ID with your photo and address), along with the intake forms we will send you.

IMPORTANT: For sending confidentialPersonal Health Information to us via the Internet, we recommend, instead of using regular e-mail or our Contact Form, that you open an encrypted e-mail account (free of charge) at ProtonMail (https://protonmail.com/), also available for the Android and Apple platforms.

Our ProtonMail encrypted e-mail address is apachecophd [AT] protonmail [DOT] com.  Please always use this address when sending us confidential, encrypted e-mail messages from your ProtonMail account.

If in person, please arrive at least a few minutes before the time of the appointment, and plan to be at our office for about one hour.

Our physical consulting offices are located between the Toronto International Airport and Centennial Park, and close to Square One, both in Mississauga, ON, Canada.  The offices are accessible, and there is free parking.

After regular work hours the office buildings are normally closed for security reasons.  Upon arrival, please approach the glass door next to the parking area.  If our assistant or specialist are not there to open the door for you, please wait and call us at the telephone number in Contact Us to advise that you have arrived.

Please send us or bring with you any insurance forms you may require us to complete.  Payment of consultation is expected at the end of each session.  If via telepsychology, by Interac transfer or, if in person, via Interac transfer, cash, or check, at the rate of _______________ per session for regular psychological therapy sessions (other rates may apply, so please ask), unless otherwise agreed upon in advance.  We do not offer credit card services.  Interac transfers are to be sent to apachecophd [AT] protonmail [DOT] com or to apacheco [AT] idpp [ DOT] org (no password required for either).

If your insurance company is paying for all or part of our services, we kindly request that you pay us for the services rendered and that you submit your claim with our receipt to your insurance company for reimbursement.

Please also call us as soon as possible, preferably at least twenty-four (24) hours prior to the day of any appointment, excluding weekends and statutory holidays,  if for any reason you cannot attend or want to reschedule, as the time indicated for your appointment is exclusively for you.

Due to the nature itself of psychological consultations, these require the utilization of the prior-appointment system and, therefore, you will be asked to pay the regular professional fees for any consultations that you do not attend to, or that you cancel the same day of the appointment.

At the beginning of our first session we will be asking you to sign required consent forms, one of them indicating that the information discussed in psychological therapy is for therapeutic purposes and is not intended for use in any legal proceedings involving you and/or your significant others, current or past.  To this effect, you will be asked to consent, now and in the future, not to subpoena Dr. Pacheco, his Assistants, and/or Associates to testify for or against any of you or to provide records in a court action.

If your consultation is intended for any legal purpose, we request that you please proceed to contact us to cancel this appointment immediately.
